DDLS Fall 2017Implementing Personalized Supports

This session will focus on strategies to identify personally-defined desired outcomes and implement personalized supports and services in a community based agency serving individuals with intellectual and other developmental disabilities. The use of individual and agency-wide qualitative and quantitative data in addition to personalized information about the preferences and life goals of those supported to identify priorities for service delivery will be presented. Practical solutions for day to day implementation of supports and services that promote personally identified outcomes, along with success stories, will also be shared.
