The Family and Support Coordination Partnership:
Making Choices, Becoming Empowered, and Supporting the Life Your Family Member Wants

As the service delivery system changes, all people with developmental disabilities receiving services through DDD will be able to choose a support coordination agency along with the services they receive. This training will provide families with the information they need about support coordination. Participants will also learn how to help their family members select a support coordination agency that will best meet their needs, work in partnership with the support coordinator throughout the person centered planning process, and identify service providers to help their family member achieve the life he or she wants.

Working in Partnership: Self-Advocates and Support Coordinators


As a person with a disability, you are able to choose the services you use and share how you want to be supported. Support Coordinators help you share information about yourself, your hopes and dreams, and the support you need through a person-centered planning process. They also help you select the service providers that will support you to be involved in your community and reach your goals. It’s important for you to learn more about what to look for and what to expect when working with a Support Coordinator and selecting services. This training will help you understand the Support Coordination process; know what to look for in a Support Coordination Agency; and learn tips and strategies for working with Support Coordinators.


Note: This training is open to people with disabilities and direct support professionals and/or family members accompanying them to provide support.

  • Individuals with disabilities and family members
  • Developmental Disability Professionals
  • Employees of DDD

All other registrants - $35 for half day courses.

Continuing Education Available: Not available for Continuing Education credit.
