The online directory was developed by Rutgers Robert Wood Johnson Medical School and Healthier Middlesex.  Please send all edits to the Guide to Maria Pellerano

Middlesex County Coordinated Assessment System

County Housing Continuum of Care (CoC) Coordinated Assessment is a countywide, coordinated process to address the needs, including permanent housing, of homeless persons. Developed by members of 27 service agencies, Coordinated Assessment (CA) matches homeless persons as quickly as possible with the most appropriate resources, according to a prioritization list based on length of homelessness and vulnerability or severity of needs. This system serves to ensure that people who need the most support can receive it in a timely manner. The CA System also provides additional information about social and health service needs and gaps, to help the community better plan assistance and identify needed resources. Middlesex County manages the 2-1-1 Homeless Hotline as the single entry point for all homeless households seeking services, regardless of age, race, color, national origin, religion, sex, disability, or household composition. If your client is homeless, call the Middlesex Homeless Hotline at 2-1-1. Your client will be screened for assistance through a two-part process (triage and full assessment).
