This guidebook for Support Coordinators, Support Coordinator Supervisors, and other plan reviewers reinforces strategies to drive a robust and informational person-centered planning. It contains an overview of each section of the PCPT and NJISP along with related information gathering tools and conversations that should be used to learn about the person. The guidebook is designed to reinforce concepts learned in training and continue to promote ways in which meaningful conversations contribute to the development of useful plans and maintains the integrity of the person-centered planning process.
