Developing a Care Plan
Many patients find that keeping a record of all medications and treatments is helpful in managing their health and remembering all there is to do. This plan can also be a helpful tool in clinic when changes are made to your/your child's healthcare regimen. You can work with your care team to review it at each clinic visit and make necessary changes.
